Assistance Applications
In order to receive assistance from Unity in Community, an application is required. Once an application is received by UIC, it is reviewed by the executive board which determines whether it can help and the extent to which assistance is given. It takes several days for an application to be reviewed and processed. Last-minute requests are not encouraged as there may not be sufficient time to process the application.

Only one application will be considered by UIC during a 365-day period.

Dental Assistance
Use this application when applying for assistance in paying for regular dental care, for fillings or extractions and for partials or complete dentures.
Dental applications are acted upon when the extent of dental work is determined by a dental office. Dentures will require some amount of repayment determined by individual situations.
